How to Receive Our Services

Supportive Home Health Care admission is open to patients ages 18 and over who are under the care of a physician, home bound, and need part time or intermittent skilled nursing care, physical therapy, or speech language pathology services, infusion therapy, parental nutritional support, laboratory services, and more.

Services may be arranged by calling Supportive Homecare directly or by contacting your attending physician, and having their office call us for your care. Our Home Care services are provided throughout Los Angeles County. Physicians, medical social workers, discharge planners, case managers, family members and patients can request our home health care services by simply calling us call at:

We will coordinate your care with your physician who supervises the services you receive. Upon receiving a referral, we will: - Obtain physician approval and all necessary authorizations - Schedule an initial assessment to determine your health needs - Coordinate your plan of care with the physician - Determine schedule of care based on your needs - Report the recovery progress to the physician. Physician consultations address: - pain and symptom management - goals and priorities for the treatment - treatment options counseling - patient and family support enhancement - improving access to and coordination of home services - orders for treatment and discharge - 30/60 days summary and discharge reports - significant changes in patient condition.

Your insurance company will pay for your home health care services. Our staff will be happy to explain the cost of the home care plan that has been coordinated for your specific needs and will work with the insurance providers to determine the home health care benefits available. If a patient needs intermittent skilled nursing care or therapy, Medicare will pay at no cost to the patient. The patient does not need a hospital stay to qualify. Medi-Cal will also pay for some home health services.

Our professional caregivers are available 24 hours a day, 7 days a week, including weekends and holidays. Supportive Home Health Care is Joint Commission on Accreditation of Healthcare Organizations accredited, California State licensed and Medicare & Medi-Cal certified.
